Each 5mL of PRIMOCET-M syrup contains levocetirizine 2.5mg and montelukast 4mg.
PRIMOCET-M is the anti-allergic combination that addresses allergic reactions and its symptoms. It is a combination of a third-generation antihistamine, levocetirizine and a leukotriene receptor antagonist, montelukast. This combination works by suppressing the activity of inflammatory mediators that are released during the allergic reactions.
Levocetirizine is an active enantiomer of cetirizine - a second-generation antihistamine. It addresses severe allergic reactions such as, itching and swelling which are commonly observed in chronic urticaria. This drug does not suppress the histamine production; however, it blocks the binding of histamine to the H1 receptor by acting as an inverse agonist of the H1 receptor. This way, levocetirizine prevents the series of allergic reactions that follow the histamine-H1 receptor binding.
Montelukast is widely known to reduce the bronchoconstriction caused by leukotrienes and thereby alleviate the symptoms of inflammation and asthma. Montelukast works as an antagonist by blocking the activity of leukotriene D4 on the cysteinyl leukotriene receptor CysLT1 present in the lungs and bronchial tubes, by binding to the receptors.

PRIMOCET-M syrup is contraindicated in patients with hypersensitivity towards levocetirizine and montelukast. It is also contraindicated in patients with hepatic and renal impairment, glucose-galactose metabolism and in patients undergoing hemodialysis.
Adverse effects of this medicine include headache, drowsiness, dizziness, GI disturbances, dry mouth, elevation in the levels of liver enzymes and fatigue.
There are no sufficient and well-controlled studies when it comes to levocetirizine and montelukast administration in pregnant women and nursing mothers. Therefore, if you are pregnant or nursing, discuss with your doctor about your condition before taking this medicine.
