Meropenem is a beta-lactam antibiotic that belongs to the class, carbapenems. This drug has broad-spectrum activity against several Gram+ve and Gram-ve bacteria. Meropenem, which is of semi-synthetic origin, is derived from thienamycin. This drug acts by inhibiting the synthesis of peptidoglycan, which is the key component of the bacterial cell wall. Unlike other beta-lactam antibiotics, meropenem is highly resistant to beta-lactamase and cephalosporinase activity.
Meropenem can penetrate through the bacterial cell wall and react with the penicillin-binding proteins (PBPs). This interaction between the drug and the PBPs of bacteria leads to inhibition of peptidoglycan synthesis. This failure in the formation of the cross linkage in the cell wall structure, eventually leads to the death of the bacterial cells. Meropenem is metabolized in the liver into an inactive metabolite which is then excreted via urine and faeces.
