Forppi is a combination of Pantoprazole (proton-pump inhibitor) and Itopride (prokinetic agent). Itopride resembles metoclopramide in its pharmacological actions (ie. antidopaminergic). Pantoprazole is classified as a proton-pump inhibitor and is used for treatment of peptic ulcer disease and gastroesophageal reflux. It effectively reduces secretion of acid from the parietal cells of the stomach. Itopride is a relatively new prokinetic agent used for treatment of gastroesophageal disease, dyspepsia and chronic indigestion. The drug inhibits neurotransmitters that inhibit gastrointestinal motility and consequently improves gastric emptying. As a result, food passes faster into the intestine and thus, there is less acid secreted for the digestive process. Itopride is also an effective agent for treatment of abdominal cramps and bloating. The combination of pantoprazole and itopride results in reduction of acid secretion and enhanced gastric motility.
Forppi Total is indicated for the treatment of gastroesophageal reflux disease and dyspepsia.
The adult oral dose of Forppi Total contains 40 mg Pantoprazole and 150 mg Itopride, taken once a day. The formula is available as capsule that is taken orally, as directed. Forppi Total can be prescribed for 4-6 weeks and re-evaluation is necessary before extending use. The formula is best taken in the morning with a glass of water. Forppi Total should not be chewed or crushed in the mouth but swallowed whole.
Pantoprazole is not recommended in children less than 12 year of age.
Itopride should not be prescribed to patients with Parkinson disease or those with active gastrointestinal bleeding.
Forppi Total is not indicated in any patient with allergy to pantoprazole or itopride or any other constituents used to make the formula.
Caution should be exercised when Forppi Total is prescribed to patients with a history of liver and kidney disease, and other gastrointestinal disorders like food poisoning, diarrhea, inflammatory bowel disease.
Long term use of Forppi Total may be associated with electrolyte changes and hence, if the treatment is prolonged, blood work should be obtained.
Prolonged used of Forppi Total is not recommended without evaluating renal and liver function every 4-6 weeks.
Forppi Total should be stored at room temperature and away from the reach of children.
Forppi Total may interact with other medications. Hence, it is important to obtain a complete drug and herbal usage history from patients before prescribing the formula.
Most common side effects reported include headache, diarrhea, dizziness and ankle swelling. These are temporary side effects that do not occur in everyone. Most of these side effects subside with time. If they persist, the patient should be re-evaluated for a change in dosage or frequency of usage.
Other rare side effects include tremor, abdominal cramps and itching
Forppi Total is not recommended for women who are pregnant and those who are breast feeding. The effects of these agents on the newborn have not yet been determined and to be on the safe side, it should be avoided in this population.
